Abstract: This review examines barriers and unmet health care needs affecting American Indian and Alaska Native (AI/AN) communities, with particular attention to access to specialty care and clinical research. The authors describe patient, provider, and institutional barriers arising from historical experiences, health system inequities, racism, limited availability of AI/AN providers, and insufficient incorporation of community leadership and cultural practices. The review highlights disparities in access to high-quality health services and the underrepresentation of AI/AN people in clinical research. It presents examples and best practices for improving access to screening, specialty care, and clinical trials and emphasizes sustained community-based partnerships and strength- and trust-based approaches to promote health equity.
