Family Care Plans Toolkit

Authors: Northwest Portland Area Indian Health Board

Publication Year: 2023

Last Updated: November 2023

Keywords: Maternal Mortality and Morbidity, Substance Use, social media toolkit

Abstract: This guide offers healthcare providers recommendations that have been demonstrated to improve outcomes for pregnant and parenting people (PPP) and their infants for three potential points of intervention: prenatally, during birth, and postnatally.

This guide also contains practical advice for developing and strengthening cross system networks of community support essential for effectively addressing the expansive ways SUDs affect the lives of AI/AN PPP, and their infants, partners, and families.

To assist these efforts, the Northwest Portland Area Indian Health Board, alongside clinicians and individuals in recovery, developed the Family Care Plans Toolkit.

This toolkit is designed to:

  • Help clinicians care for AI/AN PPP and their infants impacted by SUDs
  • Support PPP transitioning into and remaining in active recovery, and
  • Assist affected partners and families in growing stronger

The toolkit offers:

  • A guide for clinicians on improving outcomes for AI/AN PPP and their infants, partners, and families
  • A variety of handouts for AI/AN PPP and their supports
  • Eye catching posts to share on organizational social media accounts
