Pregnancy-Related Deaths Among American Indian or Alaska Native Persons: Data From Maternal Mortality Review Committees in 38 U.S. States, 2020

Abstract: The Centers for Disease Control and Prevention (CDC) conducts national surveillance to better understand the causes of pregnancy-related deaths. The Pregnancy Mortality Surveillance System (PMSS) defines a pregnancy-related death as a death during or within 1 year of the end of pregnancy from any cause related to or aggravated by the pregnancy. Medical epidemiologists review and analyze applicable vital records, and additional available data from all 50 states, New York City, and Washington, DC. Beginning in 2020, data from Puerto Rico is included in PMSS. This report uses data on pregnancy-related deaths among residents of 38 states in 2020 shared with the Centers for Disease Control and Prevention (CDC) through MMRIA by using an alternate approach for classifying AI/AN populations. This report describes pregnancy-related deaths among all AI/AN persons, which includes non-Hispanic single-race AI/AN persons, and AI/AN persons in combination with any other race or ethnicity.

Spotlight Statistics:

  • AI/AN pregnancy-related mortality ratio (PRMR) jumped from 32.0 maternal deaths per 100,000 live births in 2017-2019 to 63.4 maternal deaths per 100,000 live births in 2020.
  • In 2020, infection and mental health conditions were the most common underlying causes of pregnancy-related death among all AI/AN persons.
  • In 2020, MMRCs determined 92% of pregnancy-related deaths among AI/AN persons to be preventable.
  • In 2020, 50% of pregnancy-related deaths among AI/AN persons occurred 7–365 days postpartum.
